Women and Men 40 years and older.
Call us if you need to be screened for the following:
- Annual mammogram (if you have not had one within a 12 month period)
- Cervical screening
- Prostate screening
(If you live within California you will be referred to local services in your area)
Navigation Services (Care Coordination) we will assist you with:
- Link to mobile and local screenings
- Make appointments for mobile screenings if needed make appointments in your local area
- Follow up with clients and if requested contact medical provider
- Link to treatment services
- Link to support groups
- Provide peer-to-peer support
ADD: All servcies are offered to everyone who are 40 years and older and eligible to state funded programs.
